Additional Fees

IF "INACTIVE," ADDITIONAL FEES MAY APPLY

Explanation of Additional Fees:

  • If "Inactive" you must prepay your $125 or $150 Certification or Re-certification fee along with this registration. (i.e. Inactive is OVER 6-months late) Reinstatement is not complete without the actual filing and approval of your forms.
  • When filing your Certification or Re-Certification documents you must ALSO pay the $50 late filing fee

BEFORE YOU ENROLL

NOTE: Refresher students may attend all 4 days of the training class. However, only the following times are required:

  • Day 1 - After lunch to 5:30
  • Day 2 - All Day
  • Day 3 - All Day
  • Day 4 - Optional

Must have completed our 4-Day Training Program to be eligible.
